The district nurses are based at Auchtermuchty Health Centre and consist of 4 members of staff (working both full and part-time).  We are a team of highly experienced nurses who visit patients in their own home or in residential care homes providing care for patients and supporting family members. 

Our philosophy of District Nursing is to provide quality patient focused centred care in a variety of settings, by utilising a holistic approach to care within the community.  This is to maximise the health potential of not only the individual patient but also address the wider health care needs of various client groups and carers.

Some of the roles that we have are as follows :

• Working together with members of the Primary Care Team and Social Work.
• Promoting and maintaining health.
• Assessment, planning, implementation and evaluation of care.
• Wound care.
• Palliative and end of life care.
• Health education and screening.
• Teaching / enabling patients and family members to care for their relatives.

The District Nurse will visit you at home if you are housebound or if there is a nursing reason why a home visit is more appropriate.  She will use their skills, knowledge and experience to assess which referrals are appropriate and when a home visit is the best option for nursing a particular patient.  Your nursing needs will be assessed and discussed with you and an agreed plan of care will be recorded.  There will be continuous assessment, evaluation and adjustment of aims.

The nursing care received by the patient will have achieved the desired outcomes set by agreed aims in partnership with the patient and carers.  The patient will be advised when no further visits are required and advised to contact their District Nurse if any further nursing needs arise.

Helping your District Nurse - contact us if you will not be home to avoid a wasted visit.  Ensure that pets are safely and appropriately secure for the nurses' visit.  Please refrain from  smoking during the nurses' visit.
